Diabetes Community Conversations
Diabetes continues to impact communities across Minnesota, with disproportionate effects on populations facing systemic barriers to health. To inform stronger, community-driven solutions, the Minnesota Department of Health’s Diabetes Unit (MDH) partnered with KANC to design and facilitate a series of community conversations aimed at gathering direct input from those most affected.
CLAS Telehealth Evaluation
To promote equity in digital health care, the Minnesota Department of Human Services (DHS) partnered with KANC to evaluate the effectiveness of telehealth services provided through Culturally & Linguistically Appropriate Services (CLAS) grants. The evaluation included four CLAS grantees that were implementing or designing services that meet the National CLAS Standards.
